We are so excited that you want to volunteer at (Agape Ministries, Inc.) Danielle Inn.  We believe that our volunteers are essential to our day to day operations.

The Louisiana State Licensing Board requires us to keep on file for each volunteer the following:

• Criminal Background Check
• Physical Exam (showing no communicable diseases)
• Drug Screening Test

To volunteer at Danielle Inn, please print the following:

Volunteer application
Criminal Background Check Authorization
Health Report Form to take to your doctor.

Upon completion return to us:

1. Volunteer Application w/two referrals (one from a pastor or priest)
2. Criminal Background Check Authorization Form
3. Health Report Form filled out by your physician.
4. Drug Screening Test Results by your physician or reputable drug testing facility.
